They don't have to. Unlicensed doctors wouldn't have the same protection from civil liability that licensed doctors do.

That license acts as a protection from 90% of lawsuits.

Unlicensed doctors would have to charge ten times as much just to make up for the civil take of people itching to sue them the instant something goes wrong.

This moral relativism of yours is exactly what lets government take this freedom, then that freedom, until we have lost them all.

Does the possession of a license raise a presumption that the care was careful?

Either presumption is subject to error.

Edit: It's nice to imagine that facts are the most important thing in trials.
